Every enterprise AI procurement follows the same script: demo works, POC metrics look good, checklist gets checked, someone signs. Then six months later the team is in a room figuring out why the system produces confidently wrong outputs on real data. The evaluation was not wrong. It was answering the wrong question.
Guardrail products are a category error. LLMs fail deterministically in production not because they lack safety filters, but because the architecture treats the model as trusted when it should be treated as an untrusted proposer. The fix is structural, not commercial.
An agent demo proves capability. Production demands resilience. The four failure modes demos hide — compounding error, silent tool failures, context rot, orchestration without a protocol — and the reliability engineering that actually closes the gap.
Your APM dashboard shows 99.9% uptime. The agent is returning wrong answers. Traditional monitoring caught the wrong thing. Production AI agents fail in a mode infrastructure monitoring was never designed to detect — and the failure is invisible by design.
The model does not fail in production. The APIs the agent needs but cannot reach do. Read access takes days. Write access takes quarters. Most pilots prove reads; production requires writes. That gap is where the 70% integration failure rate actually lives. It is not a model problem.
The industry is hiring 'AI orchestrators' as if they are a new breed. They are not. The orchestrator engineer is the senior engineer, finally doing the part of the job that actually mattered all along. The title is new. The role is not.
88% of enterprise AI pilots never reach production. The failure is not the model, and it is not the data. It is the assumption that a pilot and a production system are the same kind of thing. They are not.
79% of multi-agent failures come from poor specification and coordination, not model capability. The industry is building runtime guardrails around systems that were never correctly defined to begin with. That is not an engineering investment. It is expensive procrastination.
Every enterprise team I talk to is buying more tokens to solve a problem that is not a token problem. Context window and agent memory are different things. The architecture that treats them as the same is the architecture that fails at six months.
