76% of manufacturing AI projects fail. The industry blames data quality and change management. I think the real failure is architectural — and it's invisible until something goes wrong at machine speed.
By 2027, 40% of enterprises will demote or decommission their AI agents. Not because the models failed. Because leaders conflated two completely different things: what an agent can access and what an agent should decide alone.
When an AI agent deletes your production database in 9 seconds, we call it a rogue agent. The real problem is that we granted the agent execution authority we never meant to grant. Autonomy and authority are not the same thing — and confusing them is the architectural error that is going to define this era.
AI coding agents can write the code. What they cannot do is decide what to build, catch the wrong abstraction before it compounds, or know when to stop. That is now the engineer's entire job — and it requires a completely different set of skills.
A systematic analysis of 1,600+ agent execution traces identified 14 failure modes. Most didn't come from model errors. They came from the infrastructure layer between probabilistic output and deterministic business logic — the layer most organisations haven't built.
